A novel strategy targeting myeloid cells and T cells in Renal Cell Carcinoma

Immune checkpoint blockade (ICB) targeting T cells has enhanced clinical outcomes in patients with renal and bladder cancers; however, response rates remain limited, particularly in cases of metastatic disease. To better understand the immune mechanisms underlying resistance to ICB, blood-based biomarkers in renal cancer patients categorized as either responders or non-responders to first-line nivolumab/ipilimumab immunotherapy were assessed. Key transcription factors were elevated in myeloid cells in non-responders to ICB potentially suggesting a primarily T cell focused approach may activate alternative pathways of immune resistance thus leading to limited response rates. We then tested a novel strategy targeting these transcription factors in myeloid cells in combination with ICB and observed improvement responses suggesting CpG-STAT3 inhibitors for genitourinary cancer therapy may disrupt tolerogenic signaling, restore immune cell activity and sensitivity to immune checkpoint inhibitors and/or T cell-based immunotherapies.
